Abstract:
هدف: یکی از روشهایی که در آن با استفاده از دادههای پراکنده درباره بیماریها و عوامل خطر منتسب به آنها، میتوان شاخصهایی را محاسبه کرد که در انجام مداخله برای کنترل بیماری و عوامل خطر مناسب است، محاسبه بار بیماریها است. بنابراین هدف این مطالعه محاسبه بار عوامل خطر منتسب به سرطانهای شایع سالمندان در استان مرکزی در سال 1395 بود. مواد و روشها: مطالعه حاضر یک مطالعه توصیفی ـ تحلیلی گذشته نگر بود. گردآوری اطلاعات براساس معیارهای گروههای سنی و جنسی، میزان بروز و مرگ ناشی از سرطان، میزان مرگ ناشی از تمام علل مرگ، اطلاعات مربوط به بقا، میزان بهبودی مبتلایان به سرطان و وزن ناتوانی منتسب به سرطان براساس جدول استاندارد GBD، به ترتیب از منابع مختلف شامل درگاه مرکز آمار ایران، آخرین گزارش ثبت مرگ و آخرین گزارش ثبت سرطان مربوط به سال 1395 که در سال1400 توسط وزارت بهداشت، درمان وآموزش پزشکی منتشر شده است به دست آمد. به منظور بررسی و تحلیل دادهها از نرمافزارهای Excel وDISMOD2 استفاده گردید. به منظور تحلیل دادهها از فرمولهای محاسبه بار بیماریها (DALY=YLL+YLD) استفاده شد. به منظور بررسی روایی و پایایی دادهها از تکنیک جلوگیری از ثبت کدهای غیرممکن و کدهای بیهوده استفاده شد. یافته ها: بیشترین بار سرطان پوست منتسب به مواجهه با نور خورشید (526/71)، بیشترین بار سرطان معده منتسب به مواجهه سابقه خانوادگی (79305/88)، بیشترین بار سرطان روده بزرگ منتسب به مواجهه عدم مصرف میوه (4063/18)، بیشترین بار منتسب سرطان پستان مربوط به عدم ازدواج (6488/58) و بیشترین بار سرطان پروستات منتسب به داشتن سابقه بیماریهای قبلی (7244/08) میباشد. نتیجهگیری: نتایج بار منتسب به عوامل خطر مختلف برای هر سرطان نشان داد که وجود سابقه خانوادگی ابتلا به سرطان، استعمال دخانیات، چاقی و عدم فعالیت فیزیکی در اکثر سرطانها و بار منتسب به آنها مشترک است.
Objectives: Calculating the burden of diseases is a method that can be used to calculate indicators that are suitable for intervention in disease control. The aim of this study is to calculate the burden of risk factors attributed to common cancers in the elderly in Central Province in 2015.
Materials and Methods: The present study was a descriptive-analytical study. Information was collected based on the criteria of age and sex groups, the incidence and death rate due to cancer, the death rate due to all causes of death, information related to survival, the rate of recovery of cancer patients and the weight of disability attributed to cancer. The data was obtained based on the GBD standard table, from the portal of Iran Statistics Centre, the latest death registration report and the latest cancer registration report. In order to check and analyse the data, Excel and DISMOD2 software were used. In order to analyze the data, disease burden calculation formulas were used (DALY=YLL+YLD). In order to check the validity and reliability of the data, the technique of preventing the registration of impossible codes and useless codes was used.
Results: The highest incidence of skin cancer is attributed to exposure to sunlight (526.71), the highest frequency of stomach cancer is attributed to exposure to family history (7930.88), in colon and rectal cancer, the highest frequency is attributed to the lack of fruit consumption (4063.18), in the case of breast cancer, the highest frequency is attributed to not being married (6488.58), The highest incidence of prostate cancer is attributed to having a history of previous diseases (7244.08).
Conclusion: The results of the burden attributed to different risk factors for each cancer showed that family history of cancer, smoking, obesity, and physical inactivity are common in most cancers and the burden attributed to them.
